FEES AND INSURANCE

RATES

My current rate is $250 for the 60-minute initial intake assessment and $200 for individual 45-minute psychotherapy sessions.

PAYMENT POLICIES

I accept payment by cash, check, or credit card. You are responsible for paying credit card processing fees. You are responsible for paying at the time of service for your session and any copayments unless prior arrangements have been made

INSURANCE

I am currently in network with Aetna, Cigna, Oscar, Oxford, and United Health Care.

If you do not have the insurance above, I can see you out of network. If your plan covers out-of-network mental health services, I can provide you with a monthly billing statement (often known as a “superbill”) to submit for reimbursement to your insurance company. Payment must be provided to me directly at the time of service. I will fill out forms and provide you with whatever assistance I can in helping you receive the benefits to which you are entitled; however, it is very important that you find out exactly what mental health services your insurance policy covers. Carefully read the section in your insurance coverage booklet that describes mental health services. If you have questions about the coverage, call your plan administrator.

TIPS TO UTILIZE OUT OF NETWORK BENEFITS

The best way to be absolutely sure of your benefits is to clarify with your insurance company member services line. You can find this phone number on the back of your insurance card.

Ask these questions when speaking to your insurance company about benefits:

How much of my deductible has been met this year?
What is my out-of-network deductible for outpatient mental health? (Outpatient means treatment outside a hospital.)
What is my out-of-network coinsurance for outpatient mental health?
Do I need a referral from an in-network provider to see someone out-of-network?
How do I submit claim forms for reimbursement? (Claims are forms that are sent to your insurance company to receive reimbursement for sessions you paid for out of pocket.)
